2013 Pre participation Physical Examinations

Date: Saturday, March 2, 2013
Time: 8:00 AM to 11:00 AM
The Sports Medicine Center will offer a sports pre-participation examination to
sports-active students. These exams are in accordance with the Minnesota State High
School League clearance protocol. Staff includes physicians and residents from
Orthopedics, Physical Medicine and Rehabilitation, Internal Medicine, and Family
Medicine; additionally, athletic trainers and physical therapists, NP’s, and PA’s
provide assistance with the exams.
The cost of the exam is $20 per student (cash or check only).
Two documents - The MC Authorization to Release Pre-participation Form and the MC Physical
Day Exam Form - MUST be completed by both the athlete and the parent/guardian before the athlete arrives for the exam on March 2, 2013 (bring these forms with you on the exam date).
A parent/guardian MUST accompany the athlete to this physical at Mayo Clinic Sports Medicine.
