Century Parent Directory Information and Opt-Out Form

The Century PTSA publishes a Parent Communication Directory every fall. All student families will automatically be included in the Directory unless they chose to opt out. There are two ways to opt out of the Parent Communication Directory:

• Complete the opt out form and return it to the Century office no later than Friday, September 14     OR
• Contact Abby Lindqust at the Century Main Office with your student’s full name and grade no later than Friday, September 14.  Contact Abby at 328-5101 or ablindquist@rochester.k12.mn.us.

Families choosing the opt-out option will have their information confidentially removed prior to printing.
