COVID-19 Vaccination Clinic on Friday, 5/7

There will be a Public Health vaccination clinic on Friday for people over age 16 who haven’t been vaccinated yet

 Who can be vaccinated at this clinic

·         Workers over age 16 and the people they live with

·         Residents of manufactured home parks over age 16

No ID or insurance is needed to sign up or to get vaccinated.  Immigration status will not be checked.

Date:          May 7 (Friday) from 11:30 am to 1:00 pm   

Location:   2100 Campus Dr SE (Public Health building)

Vaccine:    Pfizer (this is a 2-dose vaccine)

Cost:          Free

How people can sign up

·         Enter the date and the passphrase:

Vaccination date: 5/7/2021

Passphrase: Hotdish1

·         Fill in the requested information and answer the consent questions. 

·         Make appointments for both of the two vaccine doses. 

o   For youth between 16 and 18: a parent or guardian will need to come to the appointment with them to give permission.

Helpful hints for registering

  • It’s best if each person registers on their own separate device (computer, tablet, cell phone etc.). 
    • If this isn’t possible, each person should close the browser when they're done and have the next person click on the registration link.
  • Chrome and Microsoft Edge are the recommended browsers.

 

Please ask people to do their best to keep their appointments.  Appointments are limited and they might not be able to reschedule.
